Welcome to our Alateen Page of Documents and Forms

The 2005 AIS Board approved having these documents available on our website for your convenience. We hope that having these available will make it easier for you to help our Alateens in the area. We want to thank you ahead of time for your willingness to be of service. Our young people really need your support.

All Al-Anon Members Involved In Alateen Service are encouraged to participate in the background check process.

All members involved in Alateen Service will need to submit:

1.     Washington Area “Al-Anon Member Involved in Alateen Service” Form

2.     World Service Office “Al-Anon Member Involved In Alateen Service” Form 

All Alateen Group Sponsors and Substitutes will also need to submit:

1.     Washington State Patrol “Request for Criminal History Information” Application Form

2.     Copy of social security card

3.     Copy of WA State Driver’s License/Identification Card

4.     Signed disclosure notice 

Helpful Hints:

          1.The WSO “Al-Anon Member Involved in Alateen Service” form– the member fills out the top part of the form. The Area fills out the authorized signature part.

          2. The WA Area “Al-Anon Member Involved in Alateen Service” form- the member fill the entire thing out, except for # 4 where they will have their nominator sign.

          3. Criminal Background Disclosure Statement – applicant fills out and signs.

          4. WA State Patrol form - Area will fill out A & B and 1st line of D. Applicant fills out C and 2nd-5th lines of D.

 

Follow-up:

1.     Any incomplete packets will be returned in their entirety to the applicant.

2.     Applicants will be notified with in 10 day of receipt of the WA State Patrol background check results.

3.     If an applicant fails the Washington State Patrol background check, their packet will be returned to them in its entirety.

4.     If an applicant passes the Washington State Patrol background check, their background check application and results will be stored in a safety deposit box in accordance with the WA Area background check guidelines.
